Conventus and Navvis Partner to Help Physicians in New Jersey Excel in Value-Based Care

The New Partnership Offers Conventus Insured Physician Members Access to Navvis’ Integrated Population Health Management Capabilities with One of the Largest New Jersey Commercial Health Plans

Woodbridge, NJ and St. Louis, MO, December 3, 2020 – Conventus Inter-Insurance Exchange and Navvis, a leading population health company, are excited to announce a unique, innovative partnership to help physicians in New Jersey lower the cost of care, improve quality, ensure access, and provide a better experience for their patients. The new partnership offers Conventus insured physician members access to Navvis’ integrated population health management capabilities with one of the largest New Jersey commercial health plans. These services are provided by Navvis’ Value-Based Services Organization, which is committed to achieving a new national standard in value-based care performance. Recently, the organization announced top-tier results in Florida, scoring among the top 6% nationwide of 541 participating ACOs.

Physicians will gain access to analytics, technology, and multi-disciplinary care coordination services, as well as support to optimize their practices. This exclusive complimentary offer to Conventus primary care physician practices helps them have access to the infrastructure and support needed to successfully participate in commercial and Medicare Advantage value-based care programs through Per Member Per Month (PMPM) payments and potential shared savings. Conventus will provide hands-on assistance in coordinating and streamlining workflow processes across other value-based care programs, such as MIPS and other commercial carrier value based programs, to avoid duplication of work.

The partnership is among the first in the nation to align patient safety and risk management strategies directly with value based care.
